This form is used for various purposes such as income verifications, mortgage applications, and financial aid processes as well. 

What is IRS Form 4506-T?

The IRS Forn 4506-T which is officially called the “Request for Transcript of Tax Return,” is used to request a transcript taxpayer’s previous tax returns. A transcript is a summary of the return information, it also includes most line items from the original tax returns. In most cases, the tax return is sufficient when tax return verification is needed. 

There are different types of transcripts available through Form 4506-T:

  1. Return Transcript: The return Transcript shows more lines from the original tax as a file, it includes any accompanying form and schedules. 
  2. Account Transcript: This transcript shows the date which includes return type, marital status, adjusted gross income, taxable income, and all payments and adjustments which is made on the account.  
  3. Record of Account Transcript: The Record of account transcript combines the information from the Return Transcript and the Account Transcript. 
  4. Wage and Income Transcript: This shows data from information returns, these include Forms W-2, 1099, 1098, and Form 5498. 
  5. Verification of Non-filing Letter: Proof from the IRS that there is no record of a filed Form 1040 series tax return for the year requested.

How to Fill Out IRS Form 4506-T

Filling out IRS Form 4506-T accurately is crucial to ensure timely processing. Here’s a step-by-step guide:

1. Taxpayer Information

  • Line 1a-1b: The first step is to enter the name and Social Security Number (SSN) or individual taxpayer Identification Number (ITIN) of the primary taxpayer. 
  • Line 2a-2b: For the next step you will enter the name and SSN/ITIN of the spouse if the request pertains to a jointly filed tax return.
  • Line 3:  You will enter your current name, address, and phone number.
  • Line 4: However, if there are differences in your current address, you will have to enter the address that is shown on your last return filed. 

2. Transcript Request:

  • Line 5: If you plan to mail the transcript to a third party, you should enter the name, address, and phone number of the third party. 
  • Line 6: Enter the tax form number (1040, 1065, etc.) and the tax years or periods requested. For example, “1040 for 2022 and 2021.”

3. Type of Transcript:

  • Line 6a-6d: You will need to check the appropriate box to know the type of transcript or verification that is required.

4. Signature and Date:

  • Line 9: The taxpayer or an authorized representative must sign and date the form for authentications and If applicable, the spouse must also sign. Be sure to include the daytime phone number as well. 

Note: If the request is for a business, an authorized representative must sign, and the form may need to include Form 2848 (Power of Attorney) or Form 8821 (Tax Information Authorization).

Where to Submit IRS Form 4506-T

Based on your location you can submit Form 4506-T via mail or fax to the appropriate IRS office. Each state has a specific address and fax number which is included in the form, here is a simple step on how to submit the form. 

1. Mail

  • You will have to review the instructions on the form to find the correct mailing address for your state.
  • Now you can send the complete form to the designated address.  

2. Fax:

  • When submitting via fax use the appropriate fax number listed on the form for your state.
  • It is best to ensure that the fax transmission is successful by keeping the confirmation page.

3. Online Request:

  • You can request some transcripts online via the IRS website using the Get Transcript Online Tool.
