How to file a claim

For individuals who satisfy these conditions and wish to file a claim, there are two ways to file a claim. A claim may be mailed or filed online. In case of mailing the claim, the claimant shall fill and send a legit claim form to Claims Office Head v. Citibank, N.A., P.O. Box 301134, Los Angeles, CA 90030-1134. The envelope, where the form will be put should be stamped on or before December 20, 2024, for it to be eligible.
